Choose the test you need from our database of over 130 tests categorized according to your needs. Select the lab location that is most convenient for you from one of our thousands of partner laboratories. Pre-pay online, allowing you access to deep discounts. |

| You will receive an authorizing e-mail that provides you with instructions to download your requisition form. You'll need this form when you go to the lab for your test. Simply download and print the form, and you can walk into the location of your choice. Appointments for most locations are accepted through www.labcorp.com. Otherwise, locations work on a first come, first served basis. |

| Once your blood sample is drawn at the partner patient service center, your blood sample is sent to a nationally accredited laboratory for analysis. |

| You will receive an e-mail notifying you that your results are available for review. Please log on to secure.healthcheckusa.com to view your results.
Results are typically available for viewing within 7 business days. (Depending on the type of result ordered, in most cases, results will be available sooner). To view your results, simply log onto HealthCheckUSA.com at your convenience, choose Get Your Test Results, enter your username and password, and click Sign In. |

Once you've completed your test, just follow these simple steps to get started:
- Call for an appointment - By calling 1-800-929-2044, you'll talk with a customer service representative who will arrange a DocTalk consultation at a time that is most convenient for you.
- The doctor calls you - At the time of your scheduled appointment, the doctor will call you at the number you provided, where you will be asked for your birth date to confirm your identity.

If for any reason you need to cancel or reschedule your consultation, simply call our customer service line 1-800-929-2044 at least 72 hours in advance of your scheduled call. If you choose to cancel, we'll issue a refund for the amount of your DocTalk consultation. If you do not cancel or reschedule at least 72 hours in advance, you will still be charged for your consultation.
