| You will receive an authorizing e-mail that provides you with instructions to download your requisition form. You'll need this form when you go to the lab for your test. Simply download and print the form, and you can walk into the location of your choice. Appointments for most locations are accepted through www.labcorp.com. Otherwise, locations work on a first come, first served basis. |

| Once your blood sample is drawn at the partner patient service center, your blood sample is sent to a nationally accredited laboratory for analysis. |

| You will receive an e-mail notifying you that your results are available for review. Please log on to secure.healthcheckusa.comto view your results.
Results are typically available for viewing within 7 business days. (Depending on the type of result ordered, in most cases, results will be available sooner). To view your results, simply log onto HealthCheckUSA.com at your convenience, choose Get Your Test Results, enter your username and password, and click Sign In.
